The Clinical Nurse Liaison is responsible for providing customer service by representing the company in a competent, professional and responsive manner to increase referral volume and expand market share at one of Gallagher's hospital referral sources in the Bridgeville, PA area. The RN liaison provides GIP clinical evaluation and assessment to determine eligibility and appropriate level of care for hospice according to the conditions of participation and/or appropriate palliative or home health plans. The RN liaison organizes and provides directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

    Marketing and Education

  • Market the services and programs of Gallagher Hospice and Gallagher’s entire portfolio of care (hospice, palliative care, home health and home care). To include one on one meetings, development and provision of educational in-services and outreach efforts.
  • Build relationships by daily interactions with physicians, hospital personnel including but not limited to case managers, discharge planners and key referral sources to ensure referral/admissions by working to promote Gallagher services.
  • Meet with patients and families during hospital stay and/or prior to discharge to assess, educate and coordinate care to ensure appropriate discharge plan.
  • Coordinate care with internal intake departments and/or coordinate between agencies.
  • Monitor and report any feedback from referral sources, communities, patients or customers that may affect agency operations.
  • Maintains a commitment to the company vision and the culture of the organization embracing teamwork, creativity, support and respect.

Patient Care

1. Performs patient assessments of the clinical status of GIP patient referrals and evaluation of patient needs.

2. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).

3. Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.

4. Assesses and evaluates patient’s status by:

  • Writing and initiating plan of care
  • Regularly re-evaluating patient and family/caregiver needs
  • Participating in revising the plan of care as necessary

5. Initiates the plan of care and makes necessary revisions as patient status changes.

6. Uses health assessment data to determine nursing diagnosis.

7. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.

8. Initiates appropriate preventive and therapy nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

9. Counsels the patient and family in meeting nursing and related needs.

10.Provides health care instructions to the patient as appropriate per assessment and plan.

Communication

  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Communicates with community health related persons to coordinate the care plan.
  • Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.
  • Provides and maintains a safe environment for the patient.
  • Assists the patient and family/caregiver and other team members in providing continuity of care.
  • Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
  • Attends interdisciplinary group meetings.

Additional Duties

  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.

6. Actively participates in quality assessment performance improvement (QAPI) teams and activities.

Requirements

QUALIFICATIONS

1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in hospice care. Bachelor’s degree, with one (1) year of hospice care experience preferred. Recent experience in marketing/healthcare sales preferred.

2. Current licensure in State and CPR certification.

3. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

5. Knowledge of insurance reimbursement including Medicare and Medicaid; hospice guidelines, including county, state and federal regulations.

6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
